Field Trip Permission Slip- Grade 6 Adventure Land

Dear Grade 6 Parents/Guardians:

We are excited to announce that on June 12th, there will be a positive behavior and attendance trip to Adventureland. For this exciting opportunity, students must meet requirements with attendance and respectful behavior in the classroom and outside of the classroom.

Below are the requirements for students’ attendance and behavioral expectations.

Respectful behavior in class and on school property. Students who receive an in-school suspension (SAVE room) more than three times during the school year may be excluded from the 6th grade trip. Students who receive ONE superintendent suspension may be excluded from the 6th grade trip.

Students who are excessively late or absent from school may not be allowed to participate in the 6th grade trip.

Absent: If a student has 5 or more unexcused absences starting on May 1, 2026, they may not be permitted to participate.

Lateness: If a student has 10 or more unexcused lateness starting May 1, 2026, they may not be able to participate.

There will be NO refund under any circumstances.

All students and their guardians must sign the contract below stating that they understand the rules and requirements for all 6th grade students.

Please sign the contract below and submit it no later than Friday, May 1, 2026. Late contracts will not be accepted will not be eligible to participate in the 6th grade trip.

c) I agree that in the event of an emergency injury or illness, the staff member(s) in charge of the trip may act on my behalf and at my expense in obtaining medical treatment for my child.

d) I understand that my child is expected to behave responsibly and to follow the school’s discipline code and policies.

e) I agree and understand that I am responsible for the actions of my child. I release the school from all claims and liability that arise in connection with the trip, except if due to the negligence of school officials.

f) I understand that I am responsible for getting my child to and from the departure and return sites identified above. I understand that my child shall be accompanied by staff member(s) during the trip, including while traveling from the departure site to the destination site, and from the destination site to the return site.

g) I understand that alcoholic beverages and/or illegal drugs are prohibited and have discussed this prohibition with my child. I understand that if my child is found in possession of these substances, they will be subject to school disciplinary procedures and possible criminal prosecution.

h) I understand that students who violate the school’s discipline code may be excluded in the future by the school from participating in a trip.
